Action Plans

Health Conditions


As you are aware, asthma can be life-threatening and must be appropriately managed through the monitoring of symptoms and administration of medication as prescribed. Most, but not all, students with asthma are identified as such in his/her health record. In order to proactively support and assist your child with their medical needs while at school, we request that you notify the school nurse if your child has a diagnosis of asthma.

If your child has a diagnosis of asthma, we need to discuss their needs with you. We also need a copy of their Asthma Action Plan, medication, medication orders and parental authorization for medication administration as prescribed. Additionally, it is very important that we have information relating to students that self-carry and administer their inhaler. The Asthma Action Plan provides guidance in the event your child has asthma symptoms. Appropriate management of asthma requires use of prescribed medication as ordered by your child’s doctor. Please provide your student’s school nurse a copy of the Asthma Action Plan provided by his/her doctor. In the event you do not have an Asthma Action Plan, please contact your child’s physician to discuss if such a plan is warranted.  

We urge you to contact the school health office to discuss your child’s current needs relating to their asthma.

El asma puede poner en riesgo la vida y debe tratarse adecuadamente a través del control de los síntomas y de la administración de medicamentos según lo descrito. El registro de salud de su hijo indica que tiene asma.  Con el fin de apoyar y ayudar de manera proactiva a su hijo con sus necesidades médicas mientras está en la escuela, le solicitamos que brinde la siguiente información al enfermero de la escuela:  

  1. Si su hijo tiene un inhalador recetado, para llevar consigo y utilizar, o para administrar y utilizar bajo supervisión, informe al enfermero de la escuela. 
  2. De acuerdo con la legislación de Illinois, debemos solicitar y obtener una copia del Plan de Acción del Asma de su hijo de parte del médico, junto con la autorización de los padres acerca de los medicamentos y una orden correspondiente del médico.  El Plan de Acción del Asma brinda pautas en caso de que su hijo presente los síntomas del asma.  Brinde al enfermero de la escuela de su hijo una copia del Plan de Acción del Asma provisto por el médico de su hijo. Si no cuenta con una copia del Plan de Acción del Asma, comuníquese con el médico de su hijo para hablar acerca de la certificación de dicho plan. 

Comuníquese con la oficina de salud de la escuela para hablar de las necesidades actuales de su hijo acerca del asma.
